The impact of PPIs on gastric digestion: understanding, spotting and restoring balance
Proton pump inhibitors (PPIs) are among the most commonly prescribed medicines in general practice and gastroenterology.
They relieve reflux effectively in the short term, but prolonged use is not without consequences1: hypochlorhydria, magnesium or B12 deficiency, microbiota disturbance, a rebound effect on withdrawal, and a higher fracture risk. Each of these is something the practitioner needs to anticipate and correct.

The essential role of gastric acidity
You almost certainly know this already: for digestion to work well, the stomach has to maintain an acid pH of between 1.5 and 2.5. This environment activates pepsin so that proteins can be broken down, neutralises ingested bacteria and yeasts, and allows the absorption of key micronutrients such as iron, calcium, magnesium, zinc and vitamin B12.
Gastric acidity also acts as a signal: it stimulates the secretion of gastrin, promotes gastric emptying and prepares the release of bile and pancreatic enzymes.
Once pH rises above 4, these mechanisms are disrupted: digestion becomes incomplete, deficiencies set in and the gut flora loses its balance.
So even without obvious symptoms, a patient on long-term PPIs should be considered at risk of hypochlorhydria.
The consequences of low HCl (hypochlorhydria)

This clinical picture is often vague and misleading: it is frequently put down to “stress” or an “irritable bowel”, when it actually reflects a shortfall in gastric acid.
The consequences of long-term PPI use
Beyond the drop in acidity they cause, PPIs taken over the long term lead to several well-documented side effects.

These effects clearly illustrate the impact of PPIs on gastric digestion, well beyond reflux alone. In practice, every patient on PPIs for more than 6 months should have a deficiency work-up (B12, magnesium, iron, calcium, zinc) and a reassessment of whether treatment is still needed.
Case study: a patient on long-term PPIs
To understand the knock-on effects of long-term PPI use on gastric digestion, here is a concrete example from clinical practice.
Profile & background
A 58-year-old man, on PPIs (omeprazole 20 mg/day) for 2 years for gastro-oesophageal reflux.
Persistent symptoms: fatigue, muscle pain, bloating after meals, coated breath, loose stools.
Blood work-up: low vitamin B12, borderline serum magnesium.
Working hypothesis
Long-term PPI use has probably induced iatrogenic hypochlorhydria4.
That would explain the incomplete protein digestion, the gradual onset of micronutrient deficiencies and the disturbed gut microbiota seen in this patient.
Before starting any management, it is essential to rule out red flags (unexplained weight loss, dysphagia, haematemesis, confirmed iron-deficiency anaemia, ulcer or severe oesophagitis). If any are present, prompt referral to gastroenterology is required.
Treatment goals
Management aims to:
Taper and then stop the PPI without triggering a rebound effect.
Restore functional gastric acidity to get digestion working properly again.
Correct micronutrient deficiencies (B12, magnesium, zinc, iron).
Rebalance the gut microbiota to limit bloating, dysbiosis and recurrent infections.
A practical four-phase approach
Withdrawing a PPI must be done gradually, with digestive and nutritional support to match.
Phase 0: make it safe & inform (W0 to W1)
First step: reassure the patient and put symptomatic cover in place.
